To support data based coordination, all EMT are expected to make the EMT Minimum Data Set (MDS) daily report. (2023-09-30)
Daily Reports should be posted through this link by 8pm daily. (2023-09-30)

Steps for MDS reporting
Get the MDS Tools for EMT (Excel Program/PowerPoint)
Submit the MDS Maker (Excel program) to EMTCC by folloiwng banner link.
Submit the MDS Maker (Excel program) to EMTCC at following banner link (MDS Feedback Maker+).
Additional items:
To adopt current context, following items of the MDS has been modified from the original MDS.

File to submit:
The MDS Maker (Excel file in the tool kit)
Please submit the Excel file as it is and not delete any tabs.
Plese re-name your file as "MDS-Maker_[TeamName]_[Date] .xlsm"
eg. "MDS-Maker_Japan Disaster Releief_20220601.xlsm".
*In case there is no network, please bring the file(s) to EMTCC physically.
*In case one EMT provided care at two places, the EMT is requested to send two daily reports by places.
