EMT MDS Daily Report

EMTCC Beira

To support data based coordination by the Ministry of Health Mozambique, all EMT are requested to send the EMT Minimum Data Set (MDS) daily report to the EMTCC. (2019-03-30)

For those EMTs in operation, please ensure that the MDS is completed on a daily basis. This is critical for both surveillance and operational planning. (2019-04-08)


Steps for MDS reporting


  1. Get the MDS Tools for EMT (Excel Program/PowerPoint)

  2. Tick in the MDS tick box for every consultation

  3. Use the MDS Maker (Excel program) to make MDS Daily Report data files(CSV)

  4. Send the MDS files to EMTCC by Email ( data@mdsgateway.net ).

Files to be sent: Two types of CSV data files (IP, DR) made by the MDS-Maker.

Please send files by 8PM daily until further instructions.

In case there is no network, please bring the files to EMTCC physically.

For those EMTs who could not make the MDS daily report to MoH/EMTCC, it is strongly advised at least to fill out following three parts of the form.

        • Team Information

        • Daily Summary

        • Needs and Risks (Free text reporting)

MDS Maker (Type C)

Saved form (Excel program file itself) should be posted to "data@mdsgateway.net" by 8pm daily.


Notes

  • To adopt country specific context, MDS19 has been suggested as "Malaria"by MoH as additional item to the WHO EMT standard.

  • In case one EMT provided clinical services at two places, the EMT is requested to send two daily reports by places.

  • MDS30 has been corrected as follows: MDS30 "Major procedure (excluding MDS31)"
