This web-site is set to provide information about the EMT MDS Daily Report. (2022-04-11)
To support data based coordination by the Health Coordination Poland, all EMT are requested to send the EMT Minimum Data Set (MDS) daily report. (2022-04-11)
For those EMTs in operation, please ensure that the MDS is completed on a daily basis. This is critical for both surveillance and operational planning. (2022-04-11)

Steps for MDS reporting
Get the MDS Tools for EMT for current emergency (Excel Program/PowerPoint)
Tick in the MDS tick box for every consultation
Input data to the MDS Maker (Excel program)
Submit the MDS Maker (Excel program) at the folloiwng link.
Files to submit: The MDS Maker (Excel file)
Please submit as it is and not delete any tabs of the Excel file.
Plese re-name your file as "MDS-Maker_POL_[TeamName]_[Date] .xlsm"
eg. "MDS-Maker_POL_NATAN_20220502.xlsm".
In case there is no network, please bring the files to EMTCC physically.
Forms and Tools
Items of the MDS will be updated from 1st of November.
Please change the form from the date and submit that weekly by every Friday.
From the week, please don't submit the original form any more.(only new form will be consolidate to the feedback report)
Please get the new forms and the Excel tool (MDS Maker) at the following link.
Please submit the Excel tool (MDS Maker) with data at this website.
Please do not delete any tab of the tool (MDS Maker) to allow smooth data processing.
Teams can choose following procedures
Submit one MDS Maker with 7days of data (please specify the h:Date of activity(dd/mm/yyyy) as a last date of the reporting period )
Submit 7 MDS makers on Friday
