To support data based coordination by the Ministry of Health/EMTCC, all EMT are requested to send the EMT Minimum Data Set (MDS) daily report to the EMTCC. (2023-03)

Daily Report are requested to be posted through this link till 11:00 pm daily. (2023-03)

Steps for MDS reporting

1. Get the MDS Tools for EMT  (Excel Program/PowerPoint)

2. Tick in the MDS tick box for every consultation 

3. Input data to the MDS Maker (Excel program)

4. Submit the MDS Maker (Excel program) to EMTCC at folloiwng banner link (MDS Feedback Maker+).

Notes:

• To adopt current context,  following items of the MDS has been modified from the original MDS.

  • MDS statistics

    • 10 Acute Watery Diarrhea/Cholera(suspected/confirmed)

  • Additional 

    • 19 Malaria (suspected/confirmed)

    • 20 COVID-19 (suspected/confirmed)

File to submit:  

The MDS Maker (Excel file in the tool kit)

• Please submit the Excel file as it is and not delete any tabs.

• Plese re-name your file as "MDS-Maker_[TeamName]_[Date] .xlsm"

  • eg. "MDS-Maker_Japan Disaster Releief_20220601.xlsm".

       *In case there is no network, please bring the file(s) to EMTCC physically.

 　 *In case one EMT provided care at two places, the EMT is requested to send two daily reports by places.
